Terms And Conditions

1. This process is intended to provide confidential information to enable the Oman Air medical department/ Oman Air Clinic to aid in assessment of fitness for travel and to provide for the passenger's special needs.
2. Passengers travelling with any one of the following conditions will be requested to prepare a Medical Information Form (MEDIF) and submit it when making a reservation:

a) Passengers whose medical condition requires oxygen supply, or needs stretcher, medical escort and / or medical treatment on board the flight.
b) Passengers whose fitness for air travel is in doubt, as evidenced by recent instability, disease, treatment or surgery,
c) Carriage and use of medical equipment or instruments (note all equipment on board must be dry cell battery operated).
d) Passengers who come under any one of the categories listed as usually unacceptable to travel on MEDIF part 1, and passenger with other serious or unstable sickness/injuries.
e) Cardio-pulmonary cases as well as those requesting continuous oxygen, stretcher or incubator should enclose a recent detailed medical report with the MEDIF request. Failure to do so will delay the processing of the MEDIF.
3. MEDIF sections must be clearly filled using Block letters or a typewriter. Yes/ No boxes should be completed with a cross in the relevant box. Please ensure information is accurate and current.
4. MEDIF forms must be submitted to Oman Air no less than 72 hours and no more 10 days prior to the departure of each flight (outbound and return).
5. Oman Air must be notified immediately of any change in the patient's condition PRIOR to travel. In the event of sudden change in the passenger's (patient's) condition during the trip, Oman Air will ask passenger (patient) to obtain another medical information form to confirm the fitness to continue further air travel. Your booking continues to be subject to our fare rules/booking terms and Conditions of Carriage.
6. Fees if any, relevant to the provision of the above information and for carrier-provided special equipment is to be paid by the passenger.
7. Passengers are requested to check with the respective office for an approval prior to travel or will received auto-generated email.
8. Cabin attendants are not authorized to give special assistance (e.g. lifting) to passengers, to the detriment of their services to other passengers. Additionally, they are trained only in first aid and are not permitted to administer any injection, or to give medication.
9. I take note that if accepted for carriage, my journey will be subject to the general conditions of carriage of the carrier(s) concerned and that the carrier(s) do not assume any special liability exceeding those conditions. I am prepared at my own risk to bear any consequences which carriage by air may have for my state of health and I release the carrier, its employees, servants and agents from liability for such consequences.
10. In order to enable Oman Air to confirm my fitness to fly for my booked flights, I hereby consent for Oman Air Medical Examiner/ Assigned Doctor to disclose the necessary information to Oman Air solely for the purpose of determining my fitness to fly and to fulfil my special assistance requirements and hereby release the attending doctor from his/her obligation to maintain my medical information strictly confidential with respect to the disclosure to Oman Air, and agree to meet such physician's fees in connection therewith.
11. I reserve the right to revoke my consent to Oman Air to process my medical information at any time but recognize that this may result in my being deemed not fit to fly and being therefore denied carriage on my booked flights. If the flight was already booked, there might be cancellation fees. I agree to notify Oman Air if there is any change in the status of my medical condition before my flight
to avoid being refused for travel.
12. I have read and understood the above statement and hereby consent to the processing of my personal data/the personal data of the minor or person whose legal guardian I am.
